On 27 July 1990 this aircraft was destroyed during an NVG mission when the crew experienced a loss of tail rotor control because of a failure at the pitch beam assembly. The crew entered an autorotational descent, but the aircraft entered a right spin and impacted the ground. Of the 13 personnel on board the aircraft, 5 suffered fatal injuries and the other 7 suffered severe injuries.
