On 27 September 1996 the crew lost Tail Rotor Control on short final to the runway. The aircraft spun to the right several times, then impacted the runway in a spin and rolled onto its right side. The aircraft structure and main rotor system sustained major damage. It was later determined that the tail rotor drive shaft failed due to improper installation at the tail take off flange.
