On 16 August 1997 the Aircraft crashed during take-off from the USS Rentz (FFG-46). Aircraft spun due to suspected pilot error before the Helicopter Aircraft Commander (HAC) came on controls to try and recover the aircraft. The Tail wheel struck the Landing Signal Officer (LSO) shack before recovery and the aircraft impacted the deck edge. With only the front half of the aircraft still balanced on the flight deck the engines and rotors were secured causing the aircraft to backflip into the ocean. All 3 crew and one PAX (ships XO) were able to egress.
