On 21 July 2009 this Aircraft crashed at Fort Pickett Army Base near Blackstone, Virginia. The accident occurred during a night training flight at Castles Combat Landing Strip when the Seahawk’s rotor wash caused a tarp used by ground crews to mark helicopter-landing zones to become airborne and entangled in the tail rotor causing the aircraft to flip on its left side. The two pilots and four crew members escaped the downed helicopter with minor injuries.
