On 22 December 1986 this aircraft was on a NVG flight with a second aircraft when the two aircraft meshed rotor blades during a passing maneuver. This aircraft was total destroyed. The second Aircraft 83-23857 was rebuilt by Sikorsky Aircraft Personnel at Kimhae Air Base in South Korea and released back to service.
