On 15 March 2005 the crew experienced white-out conditions while on approach to land in a snowy, mountainous area near Buckley, Colorado and crashed. Main rotor struck tree, left main undercarriage started to plow into snow, tail rotor struck a tree and gearbox assembly separated. Main rotor struck more trees, then the ground and separated from helicopter. All four crew members suffered only minor injuries, but the helicopter suffered Class A (repairable) damage.

Apparently, the aircraft has since been repaired and is now back in service.
