On 09 December 1985 this aircraft was on a mission to locate another downed aircraft when it struck a set of High Voltage Power Lines and suffered an In-flight break up. The Main and Tail Rotor Blades separated from the aircraft and total control was lost. The aircraft impacted a tree and then came to rest along a small stream.
