On 22 June 1988 this aircraft was damaged when it stuck another UH-60A aircraft that was parked on a hospital helipad. The aircraft lost its tail rotor components and the main rotor blades broke off at the spindle assembly. The aircraft came to rest on its left side and the parked aircraft suffered damage to the tail pylon and tail rotor assembly. Tail Number of the parked aircraft is currently unknown.
