On 30 September 1990 the Aircraft crashed at Sea off the coast of Oregon. The Aircraft was deployed with the USS Crommelin (FFG-37) at the time. The USS Crommelin was heading north along the western coast off Oregon and at the time the Helicopter was flying about 600 yards behind the ship when a flight deck safety officer reported smoke from the aircraft’s right engine. While attempting to make an emergency landing on the ship the aircraft seemed to roll over and crash into the sea. After 3 days the search for the aircraft and crew was called off. About a year later the aircraft was found and recovered but the crew was never found.
