On 9 December 2004 this aircraft was destroyed when during a Night NVG approach to a Forward Arming and Refueling Area (FARP) chalk 2 (AH-64A) hit Chalk 1 (UH-60A) while attempting to land. Both pilots in the AH-64 were killed. The crew and passengers from the UH-60A received minor injuries. Both aircraft were destroyed in a post-crash fire.
