On 19 January 1991 this aircraft was conducting an NVG flight and was attempting to land at a Hospital Helipad. The helicopter struck the ground in a left turn with a nose low attitude. The aircraft was doing approximately 100 knots when it impacted the ground, it bounced and skid for about 700 feet before coming to a stop. Of the seven personnel onboard the aircraft one sustained fatal injuries.
