On 21 March 1984 this aircraft was performing a sling load operation. Sometime after the aircraft had picked up the load (an empty pallet) and departed the load swung up and hit the tail boom, it then rotated up and hit the Main Rotor System. The aircraft descended into some trees and impacted the ground. The tailboom separated, and the aircraft came to rest inverted where it burst into flames. The PIC and Crewchief were evacuated, but the Co-Pilot sustained fatal injuries.
