On 08 December 1983 this aircraft was damaged when the Hydraulic Deck Pylon separated from the aircraft and stuck the Tail Rotor System. This caused an imbalance in the Tail Rotor System, and it ultimately separated from the aircraft. The aircraft sustained major damage. Aircraft had to be sent to Corpus Christi Army Depot for repairs.
