On 30 July 2003 while executing an approach to exfiltrate a ground force from a Military Operations Urban Terrain site, the Aircraft touched down with forward momentum, and the main rotor system contacted a communications van. As a result, the main rotor system began to disintegrate, and the aircraft developed a severe vertical vibration that tore the transmission and rotor head from the aircraft.
