On 20 October 2005 while on a Night (NVG) Medevac call the Aircraft Overturned and suffered damage to the Main Rotor Blades and Tail Boom when the crew was making a landing to a dusty landing zone and struck a tactical HMMWV.
The Aircraft was sent to CCAD for repairs and was back in service by July 2007. |

On 02 February 2021 while performing annual crew evaluations the crew encountered increased precipitation and reduced visibility and initiated IIMC recovery, when the Aircraft impacted terrain below a ridgeline south of Lucky Peak outside of Boise, Idaho.
