In 1990 the Aircraft was accepted into the Air Force Inventory and assigned to the 304th Air Rescue Squadron (304th ARS) at Portland Air National Guard Base (ANGB), Oregon.

In February 1992 the Unit was redesignated as the 304th Rescue Squadron (304th RQS).

On 30 May 2002 this Aircraft crashed near the summit of Mount Hood in Oregon at a level of 10,700 feet MSL while assisting civilian authorities to rescue 3 critically injured climbers who had fallen into a crevasse on the Mountain. All crew members were able to egress with non-life-threatening injuries.

Shortly before impact, the helicopter was in a 30-foot hover at 10,700 feet MSL, over the rescue site, which was located on the southwestern flank of the mountain. The flight engineer was preparing to commence lifting one of the victims who was on a litter hooked to the aircraft hoist when the Main Rotor RPM decreased, and the helicopter began to descend. The aircraft then went into an uncontrolled right yaw, which put the pilot’s intended escape route behind him. The flight engineer cut the hoist cable. While attempting to maneuver to land the helicopter on the 45-degree slope of the mountain, the main rotor blades impacted the steeply sloping terrain. The helicopter began to roll side over side down the mountain slope. The helicopter rolled seven and a half times before coming to rest inverted, approximately 200 feet below the impact site.

This accident was videotaped by an orbiting news helicopter and shown on network TV news shows.

In 2007 after being repaired from the Mount Hood accident the Aircraft was located with the 55th Rescue Squadron (55th RQS) at Davis-Monthan Air Force Base, Arizona.

On 9 June 2010 this Aircraft callsign "Pedro 66" was shot down in Helmand province, Southern Afghanistan. The helicopter crashed in the Sangin district, and the Taliban claimed its fighters had shot down the aircraft with a rocket-propelled grenade. The aircraft was being flown to pick up a wounded British Marine in the British base in Sangin, southern Afghanistan, when it was hit in the tail rotor by a rocket propelled grenade. 5 of the 7 crew were killed, 2 were critically wounded.
