On 29 November 2006 the crashed while attempting to land on the HMAS Kanimbla off the coast of Fiji. The helicopter was carrying 10 Australian military personnel - including six SAS soldiers and four from the Townsville-based Fifth Aviation Regiment. The aircraft hit the deck hard while attempting to land and then rolled over the deck into 2000-Meter-deep water and sank. The pilot, Cpt Mark Bingley of 171 Sqn and an SAS Trooper, Josh Porter, were killed and eight occupants survived the accident. In July 2008, A Board of Enquiry reported that pilot error was the main contributing factor to this accident.
