During Operation Colombia in December 1990, which targeted Casa Verde (FARC headquarters), five Black Hawks participated in an assault along with nine other helicopters. During this operation on 09 December 1990 UH-60 FAC4102 was damaged and made a forced landing. While being evacuated to Melgar two days later (11 December 1990), it crashed three kilometers from the base, killing all seven crew members.
