On 22 July 1989 this aircraft had just departed for an NVG mission and was at an estimated altitude of 20 feet doing 70 knots when the crew got an undetermined master caution/segment light. Immediately after the aircraft stuck a large boulder in a low nose and slightly left roll attitude. After impacting the boulder, the aircraft cartwheeled one time and came to rest in an upright position. It was destroyed by the force of the impact and post-crash fire. Besides the six fatalities there was one major injury.
