On 27 February 1991 near the end of the Persian Gulf War, Army Blackhawk 78-23015, call sign "Cowboy 15" was shot down in Iraq with nine soldiers on board. It appears the aircraft was relocating the crew of UH-1 #14273 back to port when they got misoriented. Few details are truly known but the aircraft was near the Ar Rumaylah Airfield in southern Iraq when it was engaged by hostile fire and destroyed.
