On 16 March 2002 this aircraft was destroyed during a Night Vision Goggle (NVG) flight as the crew was executing an approach to a snow-covered confined area. They drifted into a tree, and the main and tail rotor blades, as well as the tail boom all separated from the aircraft. The aircraft impacted the ground nose-low and rolled inverted.
