On 26 August 1983 this aircraft was on a Maintenance Test Flight when a retention bolt for the Main Rotor Pitch Change Rods with no safety vibrated free. As a result, the Pilot's loss complete control of the aircraft and experienced an in-flight break-up. A post-crash fire destroyed most of the aircraft.
